Insurance and Scheduling

In Network benefits

Healing Together, PLC strives to serve as an advocate for clients with the following insurance plans. Therapist will confirm benefits/eligibility, submit claims and be reimbursed directly by the following health plans:

Aetna

Anthem

Blue Cross and Blue Shield

Care First

Cigna

Coventry

United Behavioral Health

Clients are only responsible for cost share as required by individual coverage for the above listed Insurance plans.

Identified patient (IP)

In order to submit a claim, an individual must be identified as the “patient.” Even though the couple or family are coming in for treatment together, only one person can be identified as the patient. It is up to the couple/family to choose which family member will be the IP. This person will be diagnosed, therefore, the IP will have more forms to complete to aid in the diagnostic process.

Out of Network benefits

 Benefits may be available for health Insurance plans not listed above. However, it is the patient’s responsibility to secure information directly from the plan.  Healing Together therapist is only able to communicate with Insurance plans listed above. For all other Health Insurance programs, payment in full is expected at each session. It may be possible to receive a reimbursement for a portion of the treatment cost by submitting a claim with the health insurance company.

Scheduling

Healing Together, PLC is accepting new patients at this time for daytime appointments. Daytime is defined as start times between 10:00 a.m. and 2 p.m. Appointments during the day are available on a regular basis. Healing Together, PLC business hours include two evenings each week. However evening appointments are more popular and remain filled by current clients. Flexibility in start time initially will lead to the best likelihood of getting an eventual evening appointment. Intake Appointment

First appointments are typically 90 minutes to allow for completion of Intake paperwork.  Please bring an insurance card and photo ID to first appointment. Some of the Intake forms are able to be accessed remotely utilizing electronic health records. New patient who are interested, simply need to be willing to share an e-mail address when scheduling the first appointment. Instructions will be e-mailed after appointment is scheduled.

Intake forms are available below for those clients who prefer not to utilize electronic health records. IF new patients elect to complete forms electronically, a link will be e-mailed to allow direct access to Intake forms and the forms below are unnecessary.

Waivers are required to be printed and signed in order to begin the therapeutic process.

Cancellation Policy

If an appointment is not canceled twenty-four (24) hours ahead of the scheduled time, Client understands and agrees that, unless contractually prohibited by their insurance carrier, Healing Together, PLC will charge $70 for the missed appointment.
